Annual Ministry Plan and Agreement for Certified PMAs

Instructions: A PMA Ministry Plan is required upon certification and shall be updated at least annually. A separate plan should be completed for each point of ministry. (i.e., If you will be serving two local congregations and the Synod, three plan will be prepared.) Please submint this form and direct your questions to the PMA Coordinator for your Area Ministry. A leave of absence request should be submitted if a Ministry Plan will not be submitted.
